Immunization Forms Required: Per the State of Wisconsin's Department of Health Services, we need to have
Immunization Record Forms on file for every student. Please note that these forms need to be submitted annually.
If you haven't submitted your child's form yet, please do so as soon as possible. The Department of Health Services will be contacting you if this form is not received.
Back-to-School Forms: Thank you to everyone who submitted their back-to-school forms! We are still in need of these forms from many of our families. Please visit
our website to print off the forms and turn them in to the School Office. The
Student Acceptable Use Policy form is especially important for students who use technology such as Chromebooks and tablets; students cannot use these devices without the form.
Pizza Lunch: Our next pizza lunch fundraiser is on
Wednesday, October 11! It is sponsored by our 8th Grade class. Please print and submit
the order form to the School Office by
Wednesday, October 4.
Sorry, no late orders will be accepted. Please note that due to rising costs, prices have increased for these lunches. We hope you continue to support our 8th Graders in their graduation fundraising efforts!
School Photos: School photo proofs need to be returned by
Wednesday, October 11 if you would like to place an order. If your child needs to have retakes done, they should bring their proofs on
October 11 for Picture Retake Day. Please contact the
School Office with any questions.
Adoration Speaker Series: Save the date for our next Adoration Speaker Series session on
Sunday, October 22 following the
11 a.m. Mass! Fr. Ken will be presenting on "How to Pray in Adoration" in the church with an invitation to participate in Adoration afterward. All are welcome!
Girl Scouts:
Girl Scouting builds girls of courage, confidence, and character who make the world a better place! At St. Charles, we currently have Troops for 1st-4th Grade. We are looking for Troop Co-Leaders and other volunteer positions to start a 5K Daisy Troop. If you are interested in having your daughter join Girl Scouts, please fill out the
